Ouch!!!!  Ya, I seen that on facebook.  While this is probably a fake pic, This happened to my brother years ago.  He was jerking a snag loose and the spoon came back in his face.  I held his cheek firm were the hook went in and pushed the barb out of his skin.  Then cliped the barb off and pulled the hook out.  We were in a remote lake in Canada and that's all I could do except to put on some antibiotics cream I had from the first aid kit.  Worked out good with not problems.  Kept on fishing.  good luck.
